Student information is considered confidential and cannot be disclosed without the written consent of an authorized person: legal parent/guardian or student that is eighteen years or older. To ensure confidentiality of student information, this form is only valid when completed by an authorized person. A valid photo ID is required upon submission of this request. All requests require at least 10 business days for processing.
